I find it a bit messy and not generic enough. The specific ideas I have in mind are: making the title field mandatory, removing the huge default "torn paper" image, introducing a type field which can be warrior or other, displaying the tracker field (and maybe also the source field?) only for warrior projects, displaying some other fields (URL [23:53] and lead) only if they're specified, and adding a field for linking to the data (IA item, ArchiveBot viewer, a section describing the data in detail if it's spread over many places, etc.).
